• Memberships have a minimum commitment period:
  – BHRT and Gut Health Plans require a 3-month minimum commitment.
  – Weight Loss Plan and Comprehensive Wellness Plan follow their stated structure and pricing guidelines.
• Early cancellations are subject to a fee equal to 50% of the remaining commitment balance. After the minimum term, memberships may be canceled with 30 days' notice prior to the next billing cycle.

• All memberships are billed monthly, beginning on the date of enrollment. Payments will be automatically charged every 30 days from the original purchase date.

• Consultation fees and lab costs are separate unless otherwise specified. Functional Foundations consultation does not include lab work. Comprehensive Wellness Plan includes the consultation, but not additional labs or supplements/medications beyond what is listed.

• Bundle Discounts are applied to active, concurrent memberships. If one of the bundled services is canceled, the discount will be adjusted accordingly in the next billing cycle.

• Membership services and benefits are non-transferable and non-refundable. Any unused benefits during the active billing cycle do not roll over unless specified in writing. A la carte services do not include chat access or plan discounts.

• Discounts, promotions, and offers cannot be combined. Membership discounts apply only to listed services, supplements, and peptides as outlined in your personalized plan.

• Functional Health members receive additional benefits at BHM:
  – 10% off all aesthetics procedures while enrolled in an active functional health membership.
